• Ensure your records are accurate and up to date .

• Ensure your Medicare or Advantage Plan requirements are being met.

• Listen to your concerns and help you achieve your health goals.

• Work with you in between your regular office visits to ensure you are receiving the best care.

• Help you get needed services to keep you healthy.

• Help you arrange for care with me and other healthcare team members.

• Help you, your family, and your other caregivers better understand your health condition(s), medications, and treatments.
